FORWARD In March 2001, the National Institutes of Health issued the following warning: "The number of Web sites offering health-related resources grows every day. Many sites provide valuable information, while others may have information that is unreliable or misleading."1 Furthermore, because of the rapid increase in Internet-based information, many hours can be wasted searching, selecting, and printing. Since only the smallest fraction of information dealing with Valtrex is indexed in search engines, such as www.google.com or others, a non-systematic approach to Internet research can be not only time consuming, but also incomplete. CHAPTER 1. STUDIES ON VALTREX Overview In this chapter, we will show you how to locate peer-reviewed references and studies on Valtrex.
The Combined Health Information Database The Combined Health Information Database summarizes studies across numerous federal agencies. To limit your investigation to research studies and Valtrex, you will need to use the advanced search options. First, go to http://chid.nih.gov/index.html. From there, select the “Detailed Search” option (or go directly to that page with the following hyperlink: http://chid.nih.gov/detail/detail.html). The trick in extracting studies is found in the drop boxes at the bottom of the search page where “You may refine your search by.” Select the dates and language you prefer, and the format option “Journal Article.” At the top of the search form, select the number of records you would like to see (we recommend 100) and check the box to display “whole records.” We recommend that you type “Valtrex” (or synonyms) into the “For these words:” box. Consider using the option “anywhere in record” to make your search as broad as possible. If you want to limit the search to only a particular field, such as the title of the journal, then select this option in the “Search in these fields” drop box. (973) 783-3935. Summary: This continuing education article familiarizes dental practitioners with the clinical presentation and treatment of herpes simplex virus (HSV). Topics include the virus and its pathogenesis, transmission, diagnosis, clinical presentation (symptoms), treatment of orolabial (mouth and lips) HSV infections, and the pharmacological (drug) treatment of orolabial HSV infections. The author stresses that practitioners should be aware of the 'asymptomatic virus shedding' phenomenon, in which a patient with HSV who does not have clinical symptoms is contagious approximately 4 percent of the time.

Valtrex
Treatment with acyclovir (Zovirax) and valacyclovir (Valtrex) is common, however, neither product is labeled for use in orolabial herpes. Other treatment strategies include counseling the patient to determine infection triggers, modify lifestyle to reduce those triggers, wear a wide brimmed hat and use a sunscreen with greater than SPF 30, and consider the use of drug agents during the prodrome (preblister) stage. A brief posttest for continuing education credits is appended to the article. 2 tables. 13 references.
Federally Funded Research on Valtrex The U.S. Government supports a variety of research studies relating to Valtrex. These studies are tracked by the Office of Extramural Research at the National Institutes of Health.2 CRISP (Computerized Retrieval of Information on Scientific Projects) is a searchable database of federally funded biomedical research projects conducted at universities, hospitals, and other institutions. Search the CRISP Web site at http://crisp.cit.nih.gov/crisp/crisp_query.generate_screen. You will have the option to perform targeted searches by various criteria, including geography, date, and topics related to Valtrex. For most of the studies, the agencies reporting into CRISP provide summaries or abstracts. As opposed to clinical trial research using patients, many federally funded studies use animals or simulated models to explore Valtrex. The following is typical of the type of information found when searching the CRISP database for Valtrex: •
Project Title: INVESTIGATION OF VALTREX IN MULTIPLE SCLEROSIS Principal Investigator & Institution: Friedman, Jacqueline; Rockefeller University New York, Ny 100216399 Timing: Fiscal Year 2001 Summary: Multiple sclerosis (MS) is a disease which significantly affects areas of the central nervous system required for normal neurological functioning. Pathological studies of patients have suggested a viral etiology for this disease, based on similarities to known viral illnesses and the pathological changes observed. Epidemiological studies also support the hypothesis of a viral cause of MS with reported outbreaks of the disease in areas such as the Faeroe Islands. We now believe that our laboratory at the Rockefeller University, as well as other research labs, have sufficient preliminary evidence to warrant a clinical trial in MS using an anti-herpes antibiotic, Valtrex, to determine if the ravages of this disease can be controlled. Specifically, we have demonstrated that the Human herpes type -6 (HHV-6) virus can be detected by immunological methods in post mortem brain specimens of MS patients (oligodendrocyte glial cells), but not in brain specimens from decedents with other diseases. In addition HHV-6 DNA has been detected in MS brain tissue by the PCR technique, but not in controls. Finally, MS patients have a high serum antibody response to the Herpes-6 virus. Valtrex is a drug inhibitory to the HHV-6 virus with a good safety profile and is FDA approved. The general plan of study is to enroll 60 patients into a two-year trial with half randomly assigned to receive the drug and the other half a

placebo. The patients will be evaluated over the trial period by two neurologists, one of whom will not know which treatment the patients have been assigned, to assess disease progression and to evaluate disability, as measured in disability rating scales. In addition the frequency of attacks will be monitored and serial brain M.R.I. scans will be obtained to determine the number of brain lesions. Blood and spinal fluid will be periodically drawn to determine the immune response to the HHV-6 virus in the Valtrex study patients and in patients not in the drug trial, but followed at Rockefeller under the protocol "Immunological Studies in MS." The National Library of Medicine: PubMed One of the quickest and most comprehensive ways to find academic studies in both English and other languages is to use PubMed, maintained by the National Library of Medicine.6 The advantage of PubMed over previously mentioned sources is that it covers a greater number of domestic and foreign references. It is also free to use. If the publisher has a Web site that offers full text of its journals, PubMed will provide links to that site, as well as to sites offering other related data. User registration, a subscription fee, or some other type of fee may be required to access the full text of articles in some journals. Finding Nutrition Studies on Valtrex The National Institutes of Health’s Office of Dietary Supplements (ODS) offers a searchable bibliographic database called the IBIDS (International Bibliographic Information on Dietary Supplements; National Institutes of Health, Building 31, Room 1B29, 31 Center Drive, MSC 2086, Bethesda, Maryland 20892-2086, Tel: 301-435-2920, Fax: 301-480-1845, E-mail:

The immune system can be weakened as a result of medications that patients receive for cancer or other serious illness or as a result of a bone marrow transplantation. Shingles in children with a weakened immune system may spread throughout the body and in some instances may be life-threatening. Acyclovir is a medication that is routinely used to treat immunocompromised children with shingles in order to prevent further spread of their shingles and to help them heal faster. This virus infects more than 95 percent of people in the United States. Most are infected in childhood, have no symptoms, and are unaware of their infection. People infected as adolescents or adults may develop infectious mononucleosis, which usually resolves completely. Once infected, most people shed the virus from their throat occasionally, and all carry the virus in their white blood cells for life. This study will determine whether the amount of virus in the blood declines or disappears with long-term valaciclovir treatment. Normal volunteers who are not taking any antiviral medicines and patients enrolled in NIH's

protocol no. 97-I-0168 (Evaluation of Valaciclovir for Prevention of Herpes Simplex Virus Transmission) or Glaxo-Wellcome protocol HS2AB 3009 at collaborating centers may be eligible for this study. Patients in the multi-center study must be about to start valaciclovir therapy for at least 1 year. All candidates must be 18 years of age or older. Study participants will be seen in clinic for about 1 hour every 3 months for a year. During these visits, they will provide information about the medicines they are taking, gargle twice with salt water and spit the fluid into a tube, and have blood drawn (no more than 8 teaspoons each visit). The blood and gargled fluid will be tested for the amount of Epstein-Barr virus and antibodies to the virus. (Blood samples will also be tested for HLA type in order to do immunologic studies in the laboratory. HLA is a marker of the immune system that is similar to blood-typing testing.) The results in people taking valaciclovir will be compared with those in people not taking the drug. People whose results show the virus has disappeared from the body will continue to be followed twice a year for 5 years with the blood and gargling tests to continue to look for evidence of virus. Also, people who develop symptoms resembling mononucleosis (e.g., enlarged lymph nodes with fever and sore throat) will be asked to have their blood tested for the virus. CHAPTER 4. PATENTS ON VALTREX Overview Patents can be physical innovations (e.g. chemicals, pharmaceuticals, medical equipment) or processes (e.g. treatments or diagnostic procedures). The United States Patent and Trademark Office defines a patent as a grant of a property right to the inventor, issued by the Patent and Trademark Office.9 Patents, therefore, are intellectual property. For the United States, the term of a new patent is 20 years from the date when the patent application was filed. If the inventor wishes to receive economic benefits, it is likely that the invention will become commercially available within 20 years of the initial filing. It is important to understand, therefore, that an inventor’s patent does not indicate that a product or service is or will be commercially available. CHAPTER 6. RESEARCHING MEDICATIONS Overview While a number of hard copy or CD-ROM resources are available for researching medications, a more flexible method is to use Internet-based databases. Broadly speaking, there are two sources of information on approved medications: public sources and private sources. We will emphasize free-to-use public sources.
U.S. Pharmacopeia Because of historical investments by various organizations and the emergence of the Internet, it has become rather simple to learn about the medications recommended for Valtrex. One such source is the United States Pharmacopeia. In 1820, eleven physicians met in Washington, D.C. to establish the first compendium of standard drugs for the United States. They called this compendium the U.S. Pharmacopeia (USP). Today, the USP is a nonprofit organization consisting of 800 volunteer scientists, eleven elected officials, and 400 representatives of state associations and colleges of medicine and pharmacy. The USP is located in Rockville, Maryland, and its home page is located at http://www.usp.org/. The USP currently provides standards for over 3,700 medications. The resulting USP DI Advice for the Patient can be accessed through the National Library of Medicine of the National Institutes of Health. The database is partially derived from lists of federally approved medications in the Food and Drug Administration’s (FDA) Drug Approvals database, located at http://www.fda.gov/cder/da/da.htm. Coffee Break: Tutorials for Biologists19 Coffee Break is a general healthcare site that takes a scientific view of the news and covers recent breakthroughs in biology that may one day assist physicians in developing treatments. Summary: This fact sheet provides people who have erythema multiforme (EM) with information on the etiology, symptoms, and treatment of this acute, self limiting, inflammatory skin eruption. EM appears in multiple forms, and because of this variation EM has been divided into the overlapping subgroups EM minor and StevensJohnson syndrome (SJS). EM minor is usually caused by the herpes virus. Additional causes are other bacterial or viral infections or reactions to medications. EM minor is characterized by a rash that may be accompanied by minor burning or itch. Round bulls eye target shaped rings are usually present on the palms. SJS causes a greater degree of damage than EM minor. SJS is characterized by large blood blisters accompanied by a red rash. A skin biopsy is needed to confirm the diagnosis. Treatment of EM begins with the identification and removal of the trigger factor if possible. EM minor usually needs

no treatment because the lesions will resolve themselves within 2 to 4 weeks. Recurrent EM caused by the herpes virus can be prevented with a continuous low dose of Zovirax or Valtrex. SJS, if identified in the early stages, can be treated with intravenous Cytoxan, pooled gamma globulin, or oral cyclosporine. Oral steroids are useful in some cases. 2 figures. Preparation Your local public library and medical libraries have interlibrary loan programs with the National Library of Medicine (NLM), one of the largest medical collections in the world. Abdominal: Having to do with the abdomen, which is the part of the body between the chest and the hips that contains the pancreas, stomach, intestines, liver, gallbladder, and other organs. [NIH] Acyclovir: Functional analog of the nucleoside guanosine. It acts as an antimetabolite, especially in viruses. It is used as an antiviral agent, especially in herpes infections. [NIH] Adverse Effect: An unwanted side effect of treatment. [NIH] Affinity: 1. Inherent likeness or relationship. 2. A special attraction for a specific element, organ, or structure. 3. Chemical affinity; the force that binds atoms in molecules; the tendency of substances to combine by chemical reaction. 4. The strength of noncovalent chemical binding between two substances as measured by the dissociation constant of the complex. 5. In immunology, a thermodynamic expression of the strength of interaction between a single antigen-binding site and a single antigenic determinant (and thus of the stereochemical compatibility between them), most accurately applied to interactions among simple, uniform antigenic determinants such as haptens.
